Who will provide my dental care?
AMERICAN DENTAL PLAN offers services rendered by general dentists and specialists. Services must be performed exclusively at ADC. This is not an insurance product. All terms subject to change without notice.
What is included in this plan?
You can choose from four types of memberships, with fees payable annually:

Individual Membership $49.00 |
Husband & Wife Membership $79.00 |
Single Parent & 1 Child Membership $79.00
(Child Must Be Born After January 1, 1990) |
Family Membership $129.00
(up to family of 6 - each additional child $25
Children must be born after January 1, 1990) |

How do I enroll? |
- Completely fill out the enrollment form, and submit it including your credit card information.
Or,
- Print the enrollment form, complete it then mail it back along with a check or money order to American Dental Plan, 35 Broadway, Hicksville, NY 11801-4266.
Or,
- Print the enrollment form, complete it with credit card information and fax it to:

| The annual AMERICAN DENTAL PLAN membership fee includes the following services at no charge: |
- Dental Exams
(two per year)
- Treatment Planning
(two per year)
- X-Rays - up to full series
(one per year)
- Cleaning - Prophylaxis
(one per year)
- Fluoride treatment for children
(one per year).
